After scheduling an appointment, you will receive a registration link from eDerm.
Login with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t login, please call the office.
If you are a new patient, we ask that you arrive 15 minutes ahead of your appointment.
Financial Policy
Co-payments must be paid in full at the time of service.
We are not responsible for knowing your insurance policy and what services are eligible for coverage. You must contact your insurance company to determine what your policy will cover.
Missed Appointment/Cancellation Policy
If you need to cancel or change an appointment, please contact our office at least 24 hours before your scheduled appointment. No-show appointments and appointments canceled with less than 24 hours’ notice may be subject to a $50.00 fee.
WE PARTICIPATE with many insurance plans, including: Aetna Commercial Health & Medicare (excludes HMO & QPOS), Alignment Medicare Advantage, Anthem BCBS (PPO and Medicare Advantage PPO), Blue Shield of CA (Commerical PPO/EPO, Commercial HMO, Medicare Advantage PPO and HMO), Cigna (OAP, PPO, HMO, Local Plus), MD Partners IPA (Imperial Health Plan & Universal Care/Brand New Day), Medicare, RR Medicare, Healthsmart (Interplan), Healthnet of CA (Commercial and Medicare), Hill Physicians IPA, United Healthcare (Commercial & Medicare, with some exceptions), Sutter Independent Physicians, Tricare (requires prior authorization). We do not accept MediCal/Medicaid.

If your plan requires a Referral to be seen by a Specialist, please obtain it from your Primary Doctor before your visit.
Our online scheduler allows patients to self-schedule up to 90 days into the future. If you are an established patient, you should see a “welcome back!” notice appear after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, please call the office after scheduling your appointment to verify your information, before filling out the registration form which will be e-mailed to you.
For Surgical or Cosmetic appointments, you will need to contact our office to schedule.
